Insurance is a contract between you and your insurance company. We will gladly file insurance claims to your insurance carrier if all necessary information is made available.
We participate with the following insurance companies: Aetna, Anthem Dental, Assurant, Cigna, Delta Dental PPO, Delta Dental Premier, Guardian, and United Concordia.
Dentemax and Dental Health Alliance Network (DHA) are dental PPO networks. We are participating providers for these 2 networks. Your dental policy, which may not be listed, may fall into either one of these networks of payees. Please contact your dental insurance carrier and ask if they are in either of these 2 networks.
